Texas Consumer Rights & Patient Information

In accordance with Texas House Bill 4224 (Section 181.105 of the Texas Health and Safety Code), this notice provides patients with clear, step-by-step instructions on how to request health care records, contact our professional licensing authority, and file a formal consumer complaint.

1. How to Request Your Health Care Records

You have the right to access, inspect, and obtain copies of your chiropractic and health care records maintained by this practice. To ensure your private health information remains secure, please follow these step-by-step instructions to make a formal request:

  1. Prepare a Written Request: Write out or print a request that includes your full legal name, date of birth, contact phone number, and a specific description of the records you are requesting (e.g., “All treatment notes and X-rays from January 2026 to present”). Your request must be signed and dated.
  2. Submit Your Request: Send your signed written request to us via one of the following official channels:

  4. Processing Timeline: In compliance with Texas law, we will respond to your valid request within 15 business days. A reasonable, cost-based fee may apply for copying, mailing, or preparing digital summaries of your records.

3. How to File a Consumer Complaint

If you wish to file a formal complaint regarding professional care, billing disputes, or health information privacy issues, you can submit an official report to state or federal oversight bodies. Follow these steps to file a complaint:

A. Filing a Complaint with the Texas Board of Chiropractic Examiners (TBCE)

  1. Access the Complaint Form: Go to the official TBCE Website or access the direct TBCE Online Complaint Portal. You can also call (512) 305-6700 to request a paper form by mail.
  2. Provide Case Details: Fill out the form accurately. Include your contact information, the name of the chiropractor or facility involved, the exact dates of the incident(s), and a clear, detailed description of your concerns. (Note: The TBCE does not accept anonymous complaints).
  3. Submit Form: Submit via the online portal, email your form to complaints@tbce.state.tx.us, or mail it physically to the TBCE compliance office address listed in Section 2.

B. Filing a Privacy or Medical Records Complaint with the Texas Attorney General

If your concern specifically relates to the handling, misuse, or illegal sharing of your medical records under the Texas Medical Records Privacy Act, you may file a consumer complaint directly with the state’s primary consumer protection agency:

  1. Visit Online Portal: Navigate directly to the Texas Attorney General Consumer Complaint Page.
  2. Select Complaint Type: Choose the “Privacy” complaint option for medical data misuse, or select the “General” consumer complaint form for alternative business and billing grievances.
  3. Complete Submission: Complete the digital questionnaire detailing your interaction with our office and submit online. Additional educational guidelines regarding your medical rights are maintained by the state under Texas Health and Safety Code § 181.103.
